Two different approaches to supporting testosterone — and why neither is automatically better.

When men explore options for low testosterone, two approaches often come up: testosterone replacement therapy (TRT) and enclomiphene. They can both be part of a physician-guided plan, but they work through very different mechanisms.

How each approach works

TRT provides testosterone from outside the body, typically as an injection or topical. Enclomiphene is an oral medication that acts on the brain’s signaling to the testes, encouraging natural testosterone production.

“The better option is the one that fits your goals, your lab results, and your health history — not the one that sounds more powerful.”

Who might discuss each option

  • Men focused on preserving fertility may discuss enclomiphene first.
  • Men with significantly low levels or specific medical circumstances may discuss TRT.
  • Some men are not candidates for either — and that is an important outcome of an evaluation too.
